Can someone explain what each line and color means on the batters hit chart?

Red means out, yellow means reached based but not a hit (error, fielder's choice, etc) and green means base hit.

I believe yellow also represents a sac fly/bunt. Correct?

Yes, yellow is also used to represent a sacrifice.
